The Ministry has become aware of your condition through several reputable sources. While you have the Ministry's sympathies having been afflicted with something at such a young age, we are also aware that you have yet to comply with the new regulation that those infected with lycanthropy must submit their name and information to the Werewolf Registry in person by November 1st. If you posses a wand, and I'm confident that you do as you attend Hogwarts, it must also be recorded on the registry.
Failure to comply with registration by the appointed time will result in consequences that may include but are not limited to immediate expulsion from Hogwarts School of Witchcraft and Wizardry, Fines up to 1,000 Galleons and confinement in Azkaban for up to a year.
I hope that you understand how gravely important it is for you to submit to the registration and I trust that you will do so by November 1st.
